When determining your monthly premium for paid continuation coverage, please consider the following:
- If only one dependent child continues coverage, the child is the paid applicant. pay the “you only” fee.
- If multiple children continue coverage, the youngest child is the paid applicant. pay the “you & child(ren)” fee.
- if only one dependent spouse continues coverage, the spouse is the cobra applicant. pay the “you (charge applicant) only” fee.
- If the spouse and children continue coverage, the spouse is the cobra applicant. pay the “you & child(ren)” fee.
- If only one surviving spouse or former spouse continues coverage, the spouse is the cobra applicant. pay the “you only” fee.
- If a surviving spouse and children or a former spouse and children continue coverage, the spouse is the cobra applicant. pay the “you & child rate.

secondary qualifying events
The initial 18-month period of Cobra coverage can be extended up to 36 months for your dependents in the event of death, divorce, or loss of dependent child status during your initial period of Cobra eligibility. for these situations, cobra coverage cannot continue beyond 36 months.
add new coverage for a dependent
You can add eligible dependents acquired through a qualifying life event (qle), such as a birth or adoption, during your annual benefit enrollment period or within 31 days after the qle.
disability extension
If you or one of your dependents is certified disabled (title ii or xvi), you may continue Cobra coverage for up to an additional 11 months. If you believe you or your dependents qualify for this extension, please submit a copy of the award notification letter from the Social Security Administration that includes the date the disability began (the date of disability must include the month, date and the year to be reviewed).
